In order to return this form to us electronically you will require Adobe Reader version 7.0 or later. The most recent version (8.1.2) is free to download from www.adobe.com/acrobat

You should type your answers in the boxes provided and when complete press ‘submit by email’. You will be given the opportunity to print out your form as well as sending it to the address that we have set-up within the form.

If you have an earlier version of Adobe Reader then you may be able to complete the form electronically but won’t be able to use the ‘submit by email’ or ‘print’ button at the end of the form. You will need to print off your completed form (using the print option within the ‘File’ menu) and either scan it in and email to us (hr@deaf-alerter.com) or fax, hand deliver, or post it to us.

You can also print the form off and complete by hand if you do not wish to complete it electronically or if your version of Adobe Reader does not allow you to type in the boxes.

If you are unable to open the form at all, or do not have a printer (if not sending electronically) please let us know and we can send it to you in an alternative format.
